Grounding Clinical AI Competency in Human Cognition Through the Clinical World Model and Skill-Mix Framework

The competency of any intelligent agent is bounded by its formal account of the world in which it operates. Clinical AI lacks such an account. Existing frameworks address evaluation, regulation, or system design in isolation, without a shared model of the clinical world to connect them. We introduce the Clinical World Model, a framework that formalizes care as a tripartite interaction among Patient, Provider, and Ecosystem. To formalize how any agent, whether human or artificial, transforms information into clinical action, we develop parallel decision-making architectures for providers, patients, and AI agents, grounded in validated principles of clinical cognition. The Clinical AI Skill-Mix operationalizes competency through eight dimensions. Five define the clinical competency space (condition, phase, care setting, provider role, and task) and three specify how AI engages human reasoning (assigned authority, agent facing, and anchoring layer). The combinatorial product of these dimensions yields a space of billions of distinct competency coordinates. A central structural implication is that validation within one coordinate provides minimal evidence for performance in another, rendering the competency space irreducible. The framework supplies a common grammar through which clinical AI can be specified, evaluated, and bounded across stakeholders. By making this structure explicit, the Clinical World Model reframes the field's central question from whether AI works to in which competency coordinates reliability has been demonstrated, and for whom.

U-Net-based Models for Skin Lesion Segmentation: More Attention and Augmentation

According to WHO[1], since the 1970s, diagnosis of melanoma skin cancer has been more frequent. However, if detected early, the 5-year survival rate for melanoma can increase to 99 percent. In this regard, skin lesion segmentation can be pivotal in monitoring and treatment planning. In this work, ten models and four augmentation configurations are trained on the ISIC 2016 dataset. The performance and overfitting are compared utilizing five metrics. Our results show that the U-Net-Resnet50 and the R2U-Net have the highest metrics value, along with two data augmentation scenarios. We also investigate CBAM and AG blocks in the U-Net architecture, which enhances segmentation performance at a meager computational cost. In addition, we propose using pyramid, AG, and CBAM blocks in a sequence, which significantly surpasses the results of using the two individually. Finally, our experiments show that models that have exploited attention modules successfully overcome common skin lesion segmentation problems. Lastly, in the spirit of reproducible research, we implement models and codes publicly available.

multimodars: A Rust-powered toolkit for multi-modality cardiac image fusion and registration

Combining complementary imaging modalities is critical to build reliable 3D coronary models: intravascular imaging gives sub-millimetre resolution but limited whole-vessel context, while CCTA supplies 3D geometry but suffers from limited spatial resolution and artefacts (e.g., blooming). Prior work demonstrated intravascular/CCTA fusion, yet no open, flexible toolkit is tailored for multi-state analysis (rest/stress, pre-/post-stenting) while offering deterministic behaviour, high performance, and easy pipeline integration. multimodars addresses this gap with deterministic alignment algorithms, a compact NumPy-centred data model, and an optimised Rust backend suitable for scalable, reproducible experiments. The package accepts CSV/NumPy inputs including data formats produced by the AIVUS-CAA software
