Biomedical image analysis competitions: The state of current participation practice

The number of international benchmarking competitions is steadily increasing in various fields of machine learning (ML) research and practice. So far, however, little is known about the common practice as well as bottlenecks faced by the community in tackling the research questions posed. To shed light on the status quo of algorithm development in the specific field of biomedical imaging analysis, we designed an international survey that was issued to all participants of challenges conducted in conjunction with the IEEE ISBI 2021 and MICCAI 2021 conferences (80 competitions in total). The survey covered participants' expertise and working environments, their chosen strategies, as well as algorithm characteristics. A median of 72% challenge participants took part in the survey. According to our results, knowledge exchange was the primary incentive (70%) for participation, while the reception of prize money played only a minor role (16%). While a median of 80 working hours was spent on method development, a large portion of participants stated that they did not have enough time for method development (32%). 25% perceived the infrastructure to be a bottleneck. Overall, 94% of all solutions were deep learning-based. Of these, 84% were based on standard architectures. 43% of the respondents reported that the data samples (e.g., images) were too large to be processed at once. This was most commonly addressed by patch-based training (69%), downsampling (37%), and solving 3D analysis tasks as a series of 2D tasks. K-fold cross-validation on the training set was performed by only 37% of the participants and only 50% of the participants performed ensembling based on multiple identical models (61%) or heterogeneous models (39%). 48% of the respondents applied postprocessing steps.

Point Tracking in Surgery--The 2024 Surgical Tattoos in Infrared (STIR) Challenge

Understanding tissue motion in surgery is crucial to enable applications in downstream tasks such as segmentation, 3D reconstruction, virtual tissue landmarking, autonomous probe-based scanning, and subtask autonomy. Labeled data are essential to enabling algorithms in these downstream tasks since they allow us to quantify and train algorithms. This paper introduces a point tracking challenge to address this, wherein participants can submit their algorithms for quantification. The submitted algorithms are evaluated using a dataset named surgical tattoos in infrared (STIR), with the challenge aptly named the STIR Challenge 2024. The STIR Challenge 2024 comprises two quantitative components: accuracy and efficiency. The accuracy component tests the accuracy of algorithms on in vivo and ex vivo sequences. The efficiency component tests the latency of algorithm inference. The challenge was conducted as a part of MICCAI EndoVis 2024. In this challenge, we had 8 total teams, with 4 teams submitting before and 4 submitting after challenge day. This paper details the STIR Challenge 2024, which serves to move the field towards more accurate and efficient algorithms for spatial understanding in surgery. In this paper we summarize the design, submissions, and results from the challenge. High-Resolution Maps of Left Atrial Displacements and Strains Estimated with 3D Cine MRI using Online Learning Neural Networks

The functional analysis of the left atrium (LA) is important for evaluating cardiac health and understanding diseases like atrial fibrillation. Cine MRI is ideally placed for the detailed 3D characterization of LA motion and deformation but is lacking appropriate acquisition and analysis tools. Here, we propose tools for the Analysis for Left Atrial Displacements and DeformatIons using online learning neural Networks (Aladdin) and present a technical feasibility study on how Aladdin can characterize 3D LA function globally and regionally. Aladdin includes an online segmentation and image registration network, and a strain calculation pipeline tailored to the LA. We create maps of LA Displacement Vector Field (DVF) magnitude and LA principal strain values from images of 10 healthy volunteers and 8 patients with cardiovascular disease (CVD), of which 2 had large left ventricular ejection fraction (LVEF) impairment. We additionally create an atlas of these biomarkers using the data from the healthy volunteers. Results showed that Aladdin can accurately track the LA wall across the cardiac cycle and characterize its motion and deformation. Global LA function markers assessed with Aladdin agree well with estimates from 2D Cine MRI. A more marked active contraction phase was observed in the healthy cohort, while the CVD LVEF group showed overall reduced LA function. Aladdin is uniquely able to identify LA regions with abnormal deformation metrics that may indicate focal pathology. We expect Aladdin to have important clinical applications as it can non-invasively characterize atrial pathophysiology. Deep Spatiotemporal Clutter Filtering of Transthoracic Echocardiographic Images: Leveraging Contextual Attention and Residual Learning

This study presents a deep convolutional autoencoder network for filtering reverberation clutter from transthoracic echocardiographic (TTE) image sequences. Given the spatiotemporal nature of this type of clutter, the filtering network employs 3D convolutional layers to suppress it throughout the cardiac cycle. The design of the network incorporates two key features that contribute to the effectiveness of the filter: 1) an attention mechanism for focusing on cluttered regions and leveraging contextual information, and 2) residual learning for preserving fine image structures. To train the network, a diverse set of artifact patterns was simulated and superimposed onto ultra-realistic synthetic TTE sequences from six ultrasound vendors, generating input for the filtering network. The artifact-free sequences served as ground-truth. Performance of the filtering network was evaluated using unseen synthetic and in vivo artifactual sequences. Results from the in vivo dataset confirmed the network's strong generalization capabilities, despite being trained solely on synthetic data and simulated artifacts. The suitability of the filtered sequences for downstream processing was assessed by computing segmental strain curves. A significant reduction in the discrepancy between strain profiles computed from cluttered and clutter-free segments was observed after filtering the cluttered sequences with the proposed network. The trained network processes a TTE sequence in a fraction of a second, enabling real-time clutter filtering and potentially improving the precision of clinically relevant indices derived from TTE sequences. Exploring Optical Flow Inclusion into nnU-Net Framework for Surgical Instrument Segmentation

Surgical instrument segmentation in laparoscopy is essential for computer-assisted surgical systems. Despite the Deep Learning progress in recent years, the dynamic setting of laparoscopic surgery still presents challenges for precise segmentation. The nnU-Net framework excelled in semantic segmentation analyzing single frames without temporal information. The framework's ease of use, including its ability to be automatically configured, and its low expertise requirements, have made it a popular base framework for comparisons. Optical flow (OF) is a tool commonly used in video tasks to estimate motion and represent it in a single frame, containing temporal information. This work seeks to employ OF maps as an additional input to the nnU-Net architecture to improve its performance in the surgical instrument segmentation task, taking advantage of the fact that instruments are the main moving objects in the surgical field. With this new input, the temporal component would be indirectly added without modifying the architecture. Using CholecSeg8k dataset, three different representations of movement were estimated and used as new inputs, comparing them with a baseline model. Results showed that the use of OF maps improves the detection of classes with high movement, even when these are scarce in the dataset. To further improve performance, future work may focus on implementing other OF-preserving augmentations.
