LLMs Can Simulate Standardized Patients via Agent Coevolution

Zhuoyun Du, Lujie Zheng, Renjun Hu et al.

Training medical personnel using standardized patients (SPs) remains a complex challenge, requiring extensive domain expertise and role-specific practice. Previous research on Large Language Model (LLM)-based SPs mostly focuses on improving data retrieval accuracy or adjusting prompts through human feedback. However, this focus has overlooked the critical need for patient agents to learn a standardized presentation pattern that transforms data into human-like patient responses through unsupervised simulations. To address this gap, we propose EvoPatient, a novel simulated patient framework in which a patient agent and doctor agents simulate the diagnostic process through multi-turn dialogues, simultaneously gathering experience to improve the quality of both questions and answers, ultimately enabling human doctor training. Extensive experiments on various cases demonstrate that, by providing only overall SP requirements, our framework improves over existing reasoning methods by more than 10\% in requirement alignment and better human preference, while achieving an optimal balance of resource consumption after evolving over 200 cases for 10 hours, with excellent generalizability. Our system will be available at https://github.com/ZJUMAI/EvoPatient.

Icon$^{2}$: Aligning Large Language Models Using Self-Synthetic Preference Data via Inherent Regulation

Qiyuan Chen, Hongsen Huang, Qian Shao et al.

Large Language Models (LLMs) require high quality preference datasets to align with human preferences. However, conventional methods for constructing such datasets face significant challenges: reliance on pre-collected instructions often leads to distribution mismatches with target models, while the need for sampling multiple stochastic responses introduces substantial computational overhead. In this work, we explore a paradigm shift by leveraging inherent regulation of LLMs' representation space for efficient and tailored preference dataset construction, named Icon$^{2}$. Specifically, it first extracts layer-wise direction vectors to encode sophisticated human preferences and then uses these vectors to filter self-synthesized instructions based on their inherent consistency. During decoding, bidirectional inherent control is applied to steer token representations, enabling the precise generation of response pairs with clear alignment distinctions. Experimental results demonstrate significant improvements in both alignment and efficiency. Llama3-8B and Qwen2-7B achieve an average win rate improvement of 13.89% on AlpacaEval 2.0 and 13.45% on Arena-Hard, while reducing computational costs by up to 48.1%.

RecGPT Technical Report

Chao Yi, Dian Chen, Gaoyang Guo et al.

Recommender systems are among the most impactful applications of artificial intelligence, serving as critical infrastructure connecting users, merchants, and platforms. However, most current industrial systems remain heavily reliant on historical co-occurrence patterns and log-fitting objectives, i.e., optimizing for past user interactions without explicitly modeling user intent. This log-fitting approach often leads to overfitting to narrow historical preferences, failing to capture users' evolving and latent interests. As a result, it reinforces filter bubbles and long-tail phenomena, ultimately harming user experience and threatening the sustainability of the whole recommendation ecosystem. To address these challenges, we rethink the overall design paradigm of recommender systems and propose RecGPT, a next-generation framework that places user intent at the center of the recommendation pipeline. By integrating large language models (LLMs) into key stages of user interest mining, item retrieval, and explanation generation, RecGPT transforms log-fitting recommendation into an intent-centric process. To effectively align general-purpose LLMs to the above domain-specific recommendation tasks at scale, RecGPT incorporates a multi-stage training paradigm, which integrates reasoning-enhanced pre-alignment and self-training evolution, guided by a Human-LLM cooperative judge system. Currently, RecGPT has been fully deployed on the Taobao App. Online experiments demonstrate that RecGPT achieves consistent performance gains across stakeholders: users benefit from increased content diversity and satisfaction, merchants and the platform gain greater exposure and conversions. These comprehensive improvement results across all stakeholders validates that LLM-driven, intent-centric design can foster a more sustainable and mutually beneficial recommendation ecosystem.
